The controller of your personal data is Vide-Optyk, based in Wrocław, at ul. Władysława Łokietka 14, 50-243 Wrocław (Łokietka Salon) and ul. Żelazna 48, 53-428 Wrocław (Żelazna Salon).

Data provided in the contact form (name, email address, phone number, message content) is processed solely for the purpose of responding to your inquiry - the legal basis is Art. 6(1)(f) GDPR (legitimate interest of the controller).
Data provided when booking an appointment (first name, last name, phone number, email address) is processed for the performance of the contract for optical services - the legal basis is Art. 6(1)(b) GDPR.
Website cookies and analytics data are collected to improve service quality and analyze traffic - only with your consent (Art. 6(1)(a) GDPR).
Contact form data is stored for the time necessary to handle the matter, and then for 12 months as protection against claims.
Appointment-related data is stored for the duration of cooperation and 5 years after its completion, in accordance with tax and accounting regulations.
You have the right to: access your data, rectify data, erase data (right to be forgotten), restrict processing, data portability and to object to processing.
If you believe that the processing of your data violates regulations, you have the right to lodge a complaint with the President of the Personal Data Protection Office (Poland).
